Responsible for utilizing the professional nursing process to provide individualized nursing care to patients across the lifespan.Cares for patients in all phases of health maintenance, preventative care, diagnosis, treatment and follow-up, rehabilitation, palliative, and end-of-life care, in hospice and home care settings. Hospice programs are uniquely regulated. Provides patient care in compliance with regulations. Delivers patient care independently in a variety of community and family settings that may have limited resources.Evaluates patients for appropriateness, and eligibility for home health and hospice as well as the safety of the care environment. Responsible for the coordination of care with an interdisciplinary team. Assesses the patient's health situation, identifies patient/family needs, and develops an individualized, interdisciplinary plan of care for the patient, family, and other care givers. Hospice education promotes pain and symptom management, the end-of-life process, spiritual, psychosocial, and emotional support while preserving the patient's dignity. A sampling of some of the specialized clinical skills that may be performed includes, but is not limited to, age related patient assessment and intervention, sterile field preparation, IV therapy, complex wound care, nutritional assessment, catheter care, fall prevention, phlebotomy, and pleural drainage. Following a competency based formal orientation, clinical competence is expected. May delegate to licensed and unlicensed personnel within their defined scope of practice as well as students and new orients.

POSITION RESPONSIBILITIES

QUALIFICATIONS

Currently licensed with the applicable State Nursing Board and/or possess multistate licensure privileges. Functions within the legal scope of practice. Valid drivers license required. Bachelors Degree in nursing preferred. Minimum of one to two years of professional nursing experience in a home health or hospice setting preferred.
